Here are some new hire documents you may find useful

Use this form so that you can properly withhold the correct federal income tax from your employees' pay. 

If you process payroll through QuickBooks desktop or QuickBooks Online you need an authorization form to directly deposit your staff's wages into their account. 

Use Form I-9 to verify the identity and employment authorization of individuals hired for employment in the United States. All U.S. employers must properly complete Form I-9 for each individual they hire for employment in the United States. This includes citizens and noncitizens. Both employees and employers (or authorized representatives of the employer) must complete the form.

Businesses that meet certain criteria are generally not required to offer health care benefits to their staff. This includes organizations with less than 50 full time or full time equivalent employees, Certain employers are required to provide this notice even if they do not offer health insurance. 

Worker's Compensation coverage requirements are determined by each state individually. Check with your State to see if it is a requirement. In instances where it is not mandatory you must provide this notice to your staff.
